Use the Module Manager tool to directly install the extension from the downloaded file, for all extensions that support Module Manager. For vtiger CRM 5.0.4, the Module Manager is not included in the base installation and you need to download and install the vtlib Extension first.
The PDF configurator is one of the most popular vtiger extention. For crm-now it is impossible to provide a free support for everyone. Most users reported that the installation is seemless. We do not know why it isn't working at some CRM and I recommend to use the vtiger forum to gain assistance. Have a look at this forum entry http://forums.vtiger.com/viewtopic.php?t=27676. Perhaps your issue has been described and solved already.
It still has a couple issues:
1) It doesn't install as a custom module, and a manual installationn is always risky for some "inexperienced" users;
2) Bank Name, Bank Account Number and other data used in the footer are hard-coded;
However, it is not difficult to fix these issues for even a low-skilled PHP developer, if it follows the instructions contained in the package.
1. Once installed looks nice and works
2. PDF design in Vtiger looks unfortunately 80ties like and is not suited to international operating companies and should defenitely be changed in a new release...
3. Seems that the PDF configurator works not with the Customer portal so your new designed invoices are not downloadable any longer in the Customer portal. This is not really good indead and there is no work-around in the Forum to get that fixed. Or you are a smart php freak or you get pissed.

I recoginzed a little bug, the naming of the pdf invoice file looks strange when there is only a contact but no organization, but that doesnt hurt the overall performance
Could be just what I was looking for but I may never know.
Installation instructions were written by and for someone a lot more advanced/smarter than I am.
If anyone figures out how to add it to an existing Vtiger setup, and can write some easy to follow set of instructions for simple folk like me, I would love to give it a try.
