Use the Module Manager tool to directly install the extension from the downloaded file, for all extensions that support Module Manager. For vtiger CRM 5.0.4, the Module Manager is not included in the base installation and you need to download and install the vtlib Extension first.

I'm using this plugin for a while now and bought two licences so far. With Outlook 2003 it really makes a good job. In comparison to the free outlook plugins this one saves you a lot of pain. It's worth every cent.
Also worth to mention: this plugin is maintained continuously, there have been a couple of bugfix releases/improvements since the first release of this plugin. Questions are answered promtly in the associated forums thread.
We use this to sync contacts with outlook, and then sync outlook to Quickbooks. So far it has been our best solution for syncing our vtiger contacts with QB.
Pro: simple to use, works well with pushing email to the CRM server, and finding the account/contact that it needs to associate with. Installation is straight forward (both client and server components). Interface is relatively clean.
Con: Syncing contacts still needs work. It appears to only sync contacts from root folder, and not from contacts that are organized into sub folders or any kind of hierarchy. This effectively means that 80% of my contacts won't sync. Also needs to remember which contacts I previously said "no" to pushing to the CRM. When syncing an email, needs an option to CHOOSE whether to push any attachments into the CRM or not, as well as to edit the message first. This is relevant if I have X messages to Y people, all having the same attachment... I don't want that going in the CRM for each one. At least make it an option.
Despite these limitations, I have no problems paying the 10 euros for the functionality that is provided so far, and this plugin worked better than the others I tried in February 2010.

Since the first posting the Plugin has been modified several times to fix reported bugs. Make sure that you keep track by the version number and install always the latest version.
Very very good add-on. I found a bug though: the plugin doesn't like accentuated chars at all. I had a contact in the CRM with a "é" in the address. Plugin was blocked and could not synchronize.
